CLAIMING CONTINUING PROFESSIONAL EDUCATION CREDITS FROM A NON-REGISTERED SPONSOR

General Instructions:

This form is used to claim credit for CPE taken from an organization that has not registered with the Board as a sponsor. You must attach to the license renewal notice, a completed copy of this form for each course.

Required Forms and Documents:
