Initial Registration for Sponsoring Continuing Professional Education

General Instructions:

This form is used by organizations and individuals who wish to register with the Board as first time sponsors of continuing education classes.

Do not use this form if you have previously registered with the Board.

Required Forms and Documents:

IMPORTANT: To ensure that you can open each of the forms below, you will need Adobe Reader 8.0 or later (Otherwise you may receive a message that the file "is damaged" or "requires a newer version of Adobe Reader"). A copy of Adobe Reader can bedownloaded at no cost. If you are using Adobe Acrobat to open the form, you must have Adobe Acrobat 8.0 or later. Forms with the notation "(fillable)" offer you the option of entering your information on-screen before printing and sending it to the Board.
